Copayment:

Network:

Preauthorization:

This is a requirement to obtain approval from the insurance company before receiving certain medical treatments or services. Preauthorization is used to ensure the treatments or services you want or need are medically necessary. In general, cosmetic treatments and procedures are not covered by general health insurance.
